- What is your annual coverage limit?
- Are there any deductibles that you should be aware of?
- How much coverage do you have per treatment?
- Is a yearly Doctor's note required by your plan?
- Does your plan offer Direct Billing or do you need to submit receipts for reimbursement yourself?
What we require from you:
- For your convenience, we kindly request to scan your insurance card during your initial visit.
- This will be securely stored for your future appointments.
- If you have a Doctor's note, we will also scan and keep it on file.
- Additionally, depending on your insurance provider's requirements, we may ask you to sign a release letter, enabling us to process your insurance claim smoothly.
Please Note:
- Please ensure you are aware of your coverage limits and keep track of your available balances. Insurance companies do not disclose balance information due to confidentiality. We regret that we cannot verify balances, statements, limits, or policy details of your benefits plan.
- If our clinic is unable to receive direct insurance payment, we will process the insurance claim to be reimbursed to you. Rest assured, we will provide you with an insurance receipt and explanation during check-out for your records.
At Goldy's Massage, we offer the convenience of insurance direct billing when the necessary technologies and insurance systems are operational. While rare, in case of system downtime, you are responsible for full payment, with an insurance receipt provided for self-submission.
Please note that Desjardins Insurance requires direct reimbursement, while Manulife requires pre-registration.
